Pregnancy Information
The Obstetrical patient forms below are forms that all new pregnancy patients will need to complete prior to their appointment. Please arrive 15 minutes prior to your appointment with your completed form. Please bring your photo ID and insurance card at the time of your visit. If you do not have an insurance card, please contact our office prior to appointment.
If you are an established Obstetrical patient and have changed your phone number, address, or insurance information, please complete the Established Obstetrical Patient Update Forms.
For insurance changes, please give us a call to be sure we participate with your new plan.

Childbirth Education:
We offer childbirth education. These classes provide the patient with an understanding of vaginal and
C-section deliveries, medications used in deliveries, postpartum, child safety, and much more. The classes are held at our Kissimmee office. Each class is approximately two hours long. Classes are held once a week for a total of five weeks. The fee that is not
covered by insurance is $75.00 for the entire course. You should sign up around your 28th week of pregnancy.
When attending the classes you will need to bring the following:

- A blanket
